Sage Practices emerged from a collective of individuals who have all, at some point, felt the weight of despair, burnout or frustration within a healthcare system that sometimes feels more fragmented than human. Yet instead of succumbing to these challenges, we chose to transform our passion for patient care and our commitment to the art of medicine into something positive, creative and regenerative.

Born from conversations between clinicians who have been seen by some as ‘quality rebels,’ Sage Practices arises out of a desire to offer careful, kind, relationship-centred care within systems that often seem governed by speed, algorithms and efficiency at the cost of real listening and individual attention. Many of us have faced moments of isolation, overwhelm and moral injury - when the work we do feels counter to the values that first drew us into caregiving. These are painful experiences, yet they are also the crucible for change. Through collaboration, creativity and mutual support, we discovered ways to bring life back to our work and purpose to our care.

We know the journey is not easy. Trying to bring change can feel risky- you might stand out, feel misunderstood or even mocked for challenging the status quo. But we also know the power of support. We found strength in community, in being seen and encouraged by others on a similar path. Sage Practices is here to offer that same sanctuary to those who are feeling depleted, disillusioned or disconnected.

We believe change is not only necessary but already unfolding. And we want to be part of that, together. Sage Practices is more than a website - it’s a home for those who need a place to rest, reflect and be inspired. A place for sharing stories, practices and ideas, where you can find others who are walking this same path. We are linked to a wider collective of organisations, pooling our resources and wisdom, seeing ourselves as the interstitium—the connective tissue that supports, cushions and protects.

As we harness new channels of communication and repurpose systems that sometimes drive us apart, our hope is to grow a movement that reconnects us to the heart of healthcare and influences meaningful change.

The African proverb reminds us: "If you want to go fast, go alone. If you want to go far, go together." So we invite you - clinicians, caregivers, staff and all humans invested in compassionate care - to join us. Together, we can move forward with courage, creativity and connection.

Sage Practices is open to anyone who cares about creating a kinder, more connected way of working in health and care. You might be:

• a large group practice experimenting with new ways of working

• a single practitioner carrying the flame for careful, kind care

• a manager or patient group finding ways to support staff and one another

• someone reaching out to your local community to build health together

• a student, community worker, or citizen who wants to nominate a practice

• or simply someone with a personal practice you’d like to share

We want to gather all kinds of Sage practices to grow a collective vision of what’s possible. When we work in and with community, we stand on solid ground and can see the subtleties that matter. Sharing our stories makes visible what often feels invisible when large systems dominate.

Every piece matters. However small it may feel, your story adds to the mosaic. If you care about careful and kind care, you belong here - and we’d love you to join us.

Because we know it’s hard to keep caring in a system that at times feels fast and furious. But the future is not yet written and change begins with those who dare to imagine something better. We are here now, so it has to be us. And we don’t have to do this alone. We are all connected. We choose the Sage plant as the symbol for our collective because it embodies the balance of science and wisdom we aim to bring to healthcare - grounded in evidence, yet deeply rooted in humanity, history and nature. The Latin name Salvia officinalis, meaning "to heal" or "saviour," reflects Sage’s long association with medicine, wisdom and longevity - qualities that resonate with our aspiration to create thoughtful and sustainable approaches to care.

Sage is more than a metaphor; it is a potent medicinal plant celebrated for its anti-inflammatory and antioxidant properties, backed by both ancient tradition and modern science. Its therapeutic compounds, like thujone and rosmarinic acid, remind us that healing isn’t confined to pills or procedures. It spans time and tradition, encompassing approaches as diverse as the human experience.

The history of Sage is woven into the fabric of global healing practices. First documented in 77 CE by Pliny the Elder, Sage has been treasured across centuries for its healing, culinary, and ritual uses. Its cross-cultural significance, seen in ceremonies and practices worldwide, highlights the enduring human recognition of the connection between health, nature and spirit. By honouring Sage, we remember that medicine has always been rooted in an understanding of and learning from the natural world.

Sage also carries mythic and symbolic resonance, like the Roman legend of the goddess Diana infusing her power into Sage leaves. These stories reflect the interplay of knowledge and humility, science and story. Just as the plant itself thrives in diverse environments, so must our approach to healthcare evolve to embrace evidence while remaining open to the wisdom of lived experience and cultural traditions.

We dream of hosting real-life gatherings and symposia- spaces where meaningful connections can deepen, and ideas can spark collaborative action. These gatherings could become crucibles for collective energy, enabling us to grow a shared voice capable of influencing policy and shaping systems that honour care, equity and sustainability.

Ultimately, Sage is about creating a sanctuary for imagination and action. It’s a place where the impossible can be dared, where small seeds of change are planted and nurtured into flourishing movements. Together, we aspire to transform the landscape of care into one where healing is possible: for our bodyminds, our spirits and the world we inhabit. Through collaboration, creativity, and connection, we believe we can contribute to something profoundly hopeful: a future in which healthcare is a true force for healing, creation, and compassion.

We are the Interstitium: a system in the body, unacknowledged and hidden in plain sight until recently but an essential support network. The interstitium is connective tissue, the cushion and the glue between all body parts performing vital connective and balancing functions much like the wetland area around river systems or mycelial networks beneath the ground.

First, take a slow breath in through your nose and slowly breathe out, stretching your out-breath for as long as you can.

It’s important to recognise that flourishing doesn’t mean avoiding challenges or hardships; it’s about cultivating small, intentional practices that help us move towards hope and possibility, even in the face of difficulty.

Start small. Even one small act of care or attention can create a ripple. Here are some gentle practices to cultivate hope and begin clearing the path to flourishing:

• Practice Active Hope: Consider engaging with Joanna Macy and Chris Johnstone’s Active Hope Training. This program helps you reconnect with purpose and resilience through structured exercises and reflections. It’s an excellent starting point to transform despair into action.

• Pause and Reflect: A simple moment of gratitude or reflection each day can shift perspective. Whether through journalling, a mindful walk, or sitting quietly with a cup of tea, these small rituals create space for possibility.

• Reach Out: Connection is key to flourishing. Talk to someone you trust, join a supportive group, or find a community where you feel seen and valued.

For practitioners, you don’t have to navigate this alone. There are resources specifically designed to support you in these moments:
